The Team take referrals from GPs, hospital teams, London Ambulance service, statutory and non-statutory services such as mental health services, London Borough of Newham and other professionals. People can refer themselves, or make enquiries on behalf of a family member or friend.
Calls are answered by an Administrator based within the team who will take basic details and pass the call to a qualified clinician who will triage the referral, assess the urgency and arrange treatment, care or provide advice.
The clinicians are knowledgeable about the range of different services and options. This helps callers to be directed to the most appropriate service to meet their needs. There is also a Duty Manager available at all times who oversees the work of the Team.
