The Community Mental Health Team (Older People) is an integrated mental health and social services team which provides psychiatric and social needs assessment, intervention and treatment. The CMHT operates as an outpatient service aims to provide a high quality service to older people with mental health problems by engaging and supporting service users, their carers and families.
The service is made up of community mental health nurses, social workers, occupational therapists, psychologists, psychiatrists and support workers. The CMHT operates a Single Point of Entry for ALL referrals to Mental Health Services for Older People.
The service takes self referrals and referral from family members, carers and health and social care professionals.

The service can provide support, treatment and interventions for a range of mental health conditions including schizophrenia, bi-polar disorder, psychosis, depression. There is a separate team who can assess older people with memory problems.
