Parts of Bedfordshire have had a community heart failure service for several years, after some hard work, Bedford and the north of the county were integrated in 2018.

The service offers education, advice, support and NICE guidance care to patients, families and other healthcare professionals who have or treat patients with a syndrome of heart failure.

Our aim is to see patients with a diagnosis with reduced ejection fraction heart failure (HFREF), however with support from the GP and Cardiologists this can be extended to those with HFPEF (preserved ejection fraction)

The service aims to reduce admissions by treating decompensated symptoms quickly, whilst educating the patient on how to monitor for symptoms and call for help earlier.

We are always striving to improve care and communication and have close links with the 2 main hospitals that cover our county, while building relationships with others. We regularly work with the CCG and its medical team to improve and assess the service that is on offer.

Referrals are accepted from one of the following:

·         GP
·         Healthcare professional (hospital)
·         Community matron
·         Community nurse
·         Practice nurse (GP)
Self referrals to this service  can only be accepted if patient has been seen previously.
