The Intermediate Care Service under normal circumstances provides short term (up to a maximum of six weeks) rehabilitation overseen by Occupational Therapists/Physiotherapists, Assistant Practitioners and support workers to patients in their own homes, having facilitated their discharge from the acute settings/bedded units.
However, please note that under Government Covid Guidance, the service is currently being used as a 'bridging service' to support an increased demand of hospital discharges. The change in process will be explained to the patient and the Next of Kin prior to the discharge occurring. The discharge will either occur with support from the Rehabilitation and Enablement Team (NHS) or the Reablement Service (Local Authority). Either way, the service provided will be the same between the two.
Some cases will be referred onto the Local Authority within a couple of days of input should it be identified that the patient will require longer term support then can be provided by either service.
