The Day Hospital is located in East Ham Care Centre. It provides comprehensive, client-centred, multi-disciplinary assessment and therapy services within a community setting.
The services are managed within Adults Services of Community Health Newham. The Services are available to residents of Newham, aged 50 years and older who suffer with a chronic or long term condition, or exacerbation, requiring therapeutic input from two or more health specialists to help them manage their symptoms/ condition, avoid unnecessary acute hospital admissions and maintain independent living for as long as possible.
Referrals to the service are discussed at the daily multi-disciplinary meeting and acceptance to the service is decided at the time.
Recent initiatives and achievements
The Day Hospital has received recognition of their work on the production of a poster highlighting Accountability. The Day hospital achieved “Commended Placement of the Year Award” from City
University in recognition of their work with pre-registration nursing students.
Day Hospital has a regular analysis of patient satisfaction.
Provision and the routine Day Hospital and Falls Prevention Service through the use of patient questionnaires.
Positive responses have been received for both services which continue to grow in strength.
Falls Prevention Clinic
The Falls Prevention Clinic (FPC) is an integrated Consultant led multidisciplinary service whose aim is to investigate the causes of falls, reduce its incidence and also of injury following falling.
The multidisciplinary team consists of doctors, nurses, physiotherapists, occupational therapists and administrative staff members. The Service is aimed at patients who must be 65 years and over, a resident of Newham and be registered with a Newham GP. They must also have/ had 1 or more falls over the past 6 months.
The multidisciplinary team carries out a holistic falls assessment and formulates the appropriate care treatment and rehabilitation programme in agreement with the patient.
Falls prevention services also promote awareness of contributory fall risk factors by providing health awareness programmes on lifestyle and activities for healthy ageing.
