Once a referral is received and processed by one of our friendly admin team members you will be sent a text message (if permission is given) with a link to book your initial telephone appointment. Alternatively, if you prefer, you can contact us on 01234 880400 to speak to a member of the admin team will take your details and book you in for an assessment with a therapist.

What happens next?
As you approach the date of your assessment, you will receive a text reminder (if permission has been given) for the appointment, as well as an email (if persmission is given) with a web-based version of the questionnaires above. This asks you to reflect on how you have been feeling over the previous two weeks and forms part of the assessment. You will simply need to place a tick in the appropriate boxes and submit the questionnaire. Your answers will be automatically updated to your file ready for your assessment with the therapist.
Assessment
The therapist will call you, at the date and time that you agreed, to complete the assessment. The assessment can take up to 45 minutes and you should be somewhere quiet and private to take the call and not be driving.
If you are unable to attend your appointment please contact us as soon as possible so that we can offer your appointment to another patient. If you do not attend (DNA) two appointments or you cancel three appointments this will result in your case is closed and you will be discharged from the service.
At the end of the assessment, the therapist will agree on a date and time with you for them to call you with an outcome and to discuss the immediate options available to you.
