Please note we are running the service differently in line with Government Covid-19 Guidance to support an increased demand of hospital discharges. Due to this, it is likely that your intervention will be no longer than 2-3 weeks with the Intermediate Care Team.

We are a multi-disciplinary team that includes Occupational Therapists, Physiotherapists, Therapy Assistants and Rehab Support Workers. We work with acute hospitals or community rehab beds to support your safe discharge and provide a seamless rehabilitation pathway. This service provides for anyone over the age of 18 years where there have been rehabilitation goals identified by the acute therapists.

We provide short term (up to a maximum of six weeks) rehabilitation to encourage and enable you to regain independence in daily living activities as well as improve your wellbeing. Our aim is to enable patients to remain in the community rather than be admitted to hospital or long-term care. A holistic assessment of your needs will be completed and individualised rehabilitation plans will be devised based on patient-centred goals. These may include

Exercise plans to improve muscle strength, mobility and balance
Personal Care
Meal Preparation
Your equipment needs will be assessed for and reviewed once issued. This equipment will be issued on a free, short-term loan from our equipment company Millbrook Healthcare. Should you wish to return any equipment, please call Millbrook on 0333 003 8074. This collection will then be arranged at a time that is convenient for you.

We will actively involve you and your NOK (where requested) when ongoing support needs are identified, we will signpost and support you towards other organisations or professionals. If it is identified that you would benefit from a long term package of care, a referral will be made with your consent to the Local Authority for an assessment, for which you will be financially assessed. In the interim, you may be transferred to a care provider service to support your care needs. This will be provided for a maximum of 5 weeks whilst your long term care needs are fully assessed and a package of care is sourced. You will receive your day to day support by the care provider whilst remaining under the care of the Intermediate Care service. 

The Intermediate Care service is part of Bedfordshire Community Health Services (BCHS)
