Home Treatment Team City & Hackney

We provide assessment and treatment to people who are experiencing a mental health emergency of a nature or severity that would otherwise require admission to inpatient services. 

The team provides prompt, intensive support to people at the time they most need help, aiming to avoid further deterioration and alleviate distress as quickly as possible.

Where appropriate, this service can provide an alternative to hospital admission, enabling people to receive treatment and care in their own home. By remaining in their own community, the disruption to their lives and the lives of those caring for them is minimised.

Referrals are accepted from secondary mental health services.

The Home Treatment Team does not accept direct GP or self-referrals. If you or someone you support needs urgent mental health help, contact the City & Hackney Crisis Assessment Team: call NHS 111 and select option 2, or walk in to the Assessment Hub at the Raybould Centre. 

Referal pathway 

  1. Person in crisis - Self, family, or a GP with a concern
  2. Crisis assessment team - NHS 111, option 2 (24/7) or the walk-in hub, 8am–8pm
  3. Assessment & triage - Trained crisis staff assess need and risk

  4. Home Treatment Team - Intensive home-based care, referred internally from secondary mental health services

     

We treat all mental health conditions. 

What we can't help with: housing
The Home Treatment Team isn't able to resolve housing problems. If housing is part of what's going on, the page should redirect clearly rather than leave people guessing:

PALS (Patient Advice and Liaison Service)
Advice, information and support if you have concerns, or don't know where to turn — available in Bengali, Hindi, Chinese, Turkish, Somali and Urdu.
