We understand that caring for someone struggling with their mental health can be challenging, and we are here to support you.
The form below is for family members, friends, and carers of adults experiencing mental health difficulties. You can also complete this form if you are a parent or guardian of a child who may be a young carer—meaning they are affected by an adult’s mental health challenges.
If multiple adults or children need support, please submit a separate form for each individual.
How We Process Your Referral
- All referrals are reviewed daily between 09:00 and 10:00, Monday to Friday.
- If you need help completing the form or have any questions, please call or message us at 07435 733 581. Our team is happy to provide guidance and support.
- Our working hours are Monday to Friday, 09:00 – 17:00.
- Once we receive your referral, we will contact you within five working days.
- Please note that the information you provide will be securely held by the organisation offering support.
What Happens Next?
Once your form is reviewed, the appropriate support team will reach out to you:
- Young Adult Carers (18-25): Our Young Adult Carer Coordinator, Ben, will be in touch.
- Children & Young People (under 18): A member of the Young Carers service will contact you.
- Caring for an Adult Receiving ELFT Services: An ELFT Carer Support Worker will call you.
- Caring for an Adult Not Receiving ELFT Services: A team member from Hackney Carers Centre will contact you.
- Supporting Someone with a Substance Misuse Issue: John, the Carer Coordinator at Turning Point, will get in touch.
