The team has been recognised for its innovative work in preventing unnecessary hospital admissions and reducing repeat admissions. Through proactive care, early intervention, and close collaboration with people with learning disabilities, their families, and carers, the team has demonstrated a compassionate and community-focused approach to care. This work has helped improve outcomes, prevent crises, and promote wellbeing within local communities.
