Newham Transitional Practice (NTP) has reached its 25th anniversary, celebrating a quarter of a century supporting patients who face barriers to accessing traditional primary care services.
NTP delivers comprehensive GP care for individuals who are unable to register with a standard GP service. These groups include individuals experiencing homelessness, refugees, and those without a fixed address, who often have complex, unmet health needs.
The practice supports a registered list of almost 5,000 patients.
The team operates across two sites, The Centre Manor Park and Vicarage Lane Health Centre in Stratford, providing a full GP offer through a flexible, inclusive, and trauma-informed approach.
Since joining ELFT in 2011, NTP has been a national exemplar of equitable healthcare for excluded groups.
Key achievements include:
- Supporting thousands of patients unable to register with mainstream GP services
- Receiving a Good CQC rating, with Outstanding recognition for care of vulnerable patients
- Hosting Big Health Days, combining flu jabs, health checks, dental care and wellbeing services for the local population
- Winning the Improving Value Award at the ELFT Staff Awards 2023
- Receiving the Non-Clinical Team Award from the Royal College of General Practitioners (RCGP) in 2024
- Having staff recognised nationally through shortlisting in the BAME Health and Care Awards in 2025, (Sihle Malapela for ‘Nurse of the Year’ category)
- Achieving the top results across Newham in the GP Patient Survey
Looking ahead, in line with ELFT's commitment to health equity and integrated care, NTP plans to:
- Increase outreach and strengthen collaboration with local organisations to reach those not yet in care
- Enhance continuity and quality of care through closer integration with PCN partners
- Explore digital health tools to improve care coordination, patient access and engagement
- Organise more Big Health Day events to support the local community
