Pathway App Team Win Best Digital Practice Award
Congratulations to the Pathways App Team have won the Chief Allied Health Professions Officer Award (CAPHO) 2026 for Best Digital Practice. The Pathways App team are a group of occupational therapists(OTs) and service users based at the John Howard Centre Forensics Unit. The app was created at the Centre to help service users to achieve their goals while staying in hospital and to make care pathways visible, measurable, and empowering for both patients and staff. Led by a small, OT-led clinician team supported by the Quality Improvement team, it was co-designed with service users from the outset. Since its launch, patients have reported a clearer understanding of their progress, greater confidence in their recovery, and a stronger sense of motivation and direction towards their recovery pathway. This has enabled staff and patients to develop positive relationships, so that progress can be monitored in real time.
SAFERR QI Team - Highly Commended in HSJ Safety Awards
A staff wellbeing initiative has been highly commended at the HSJ national safety awards. The SAFERR initiative was created in Tower Hamlets Community Health Services to improve staff safety and improve the culture around incident reporting. They wanted to create an environment where staff felt more confident and supported to report incidents, recognising reporting as an opportunity for learning and improvement. It stands for
S: Stop
A: Ask for help
F: Find out more
E: Everyone needs to know
R: Report
R: Record
Staff have said that since the adoption of SAFERR, they feel issues are acknowledged, they feel better prepared and more aware of risks. The team were also a finalist in the Developing a Positive Safety Culture Award category.
Newham CAMHS DBT Highly Commended
Newham's CAMHS Dialectical Behaviour Therapy team were highly commended in the award category: Harnessing a Human Factors Approach to Improve Patient Safety Award. The team found that young people with emotional, behavioural and interpersonal dysregulation who self-harmed or displayed suicidal behaviours were often misunderstood, and the impact on mental health staff was often significant.
Dr Ana Pascual Sanchez, Clinical Psychologist and CAMHS DBT Lead & CAMHS Trustwide Audit Lead said, "We were recognised for the value we put in staff's emotions. There is often a gap in patient care when we address patient safety, when staff may feel unsafe and uncontained. The DBT approach of the consultation space and its agreements is essential to improve staff wellbeing and patient safety, because we are all human, and we all feel."
*Bedford’s Path 2 Recovery team was also shortlisted for an HSJ Safety Award for their work with a local Primary Care Network combining addiction recovery work and practice pharmacist support for people dependent on prescribed medication. After four years, almost all cases seen remain on lower doses or off the medication entirely.
