Community Mental Health Teams - Older People
The Community Mental Health Team (Older People) is an integrated mental health and social services team which provides psychiatric and social needs assessment, intervention and treatment. The CMHT (OP) aims to provide a high quality service to older people with mental health problems by engaging and supporting service users, their carers and families. The CMHT (OP) operates a Single Point of Entry for ALL referrals to Mental Health Services for Older People.
NB. From Monday 4 November, patients with appointments will be seen at Vivienne Cohen House located on Homerton High Street. The service has moved temporarily for two weeks to enable works to be carried out. All phone lines have been transferred so service users and referrers will be able to contact the team in the usual way. People with appointments have been contacted to advise them of the change of location.
Via GPs, District Nurses, Health Visitors, Social Workers
The service is multi-disciplinary, staffed by community mental health nurses, social workers, an occupational therapist, psychologists, psychiatrists and support workers.
