Advanced Care Planning Team (Tower Hamlets Community Health Services)
The Advanced Care Planning team work alongside the district nurses, supporting them to care for patients on their caseloads who may be in the last months, weeks or days of their lives. They support the nurses to have conversations with patients, their families and others who are important to them around what matters to them as they approach the end of life. They offer patients the opportunity to talk about their wishes for their future care, including where they would like to be cared for and where they would like to die.
Mile End Hospital
The team support with co-ordinating care for patients who may be nearing the end of their lives, working in close partnership with GPs, social workers, carers and the hospice. They work with the district nurses to make sure that people in the last weeks of their lives are offered care and support in line with their wishes and preferences. They support people who are dying to be in their preferred place with support available for both them and their families.
The team includes a learning disability nurse who works with patients with a learning disability who may be nearing the end of life. The Nurse supports people with learning disabilities to be able to make decisions about the care and support they would prefer, in the last months of their lives.
Nurse also assists with making reasonable adjustments for patients with learning disabilities, so that they can have equitable access to end of life care, and raises awareness across the service of the needs of people with learning disabilities in relation to end of life.
The ACP team work in partnership with the District Nurses, supporting patients on the District Nurse caseload. Please refer to the District Nurses and request support with advance care planning in the referral.
