What the Service Does
The Rehabilitation and Recovery team provides personalised, community-based care that combines clinical, social, and practical interventions to promote recovery, stability, and independence. This includes:
- Medication management, including specialist input on complex treatments like clozapine
- Psychological support, including trauma-informed and recovery-focused care
- Daily living and vocational support, helping with skills like cooking, budgeting, and accessing education or work
- Physical health monitoring, including support for GP visits and access to health checks
- Support with accommodation, including placement reviews, transitions, and liaison with accommodation providers
- Involvement of families and carers, with opportunities for joint planning and support
The team works closely with GP's, supported housing, and other mental health services to provide joined-up care.
