Diabetes Specialist Nursing Service - Newham

• Complete the appropriate referral form.
• Submit the referral via the patient's GP.
• Email completed referrals to Newham.Pbs@nhs.net.
• For urgent referrals, please contact the service by telephone.

What Happens After Referral?

1. Your GP submits a referral to the Diabetes Specialist Nursing Service.
2. The referral is reviewed by a member of the clinical team.
3. You will be contacted to arrange an appointment or education programme, where appropriate.
4. A personalised plan of care will be agreed with you.
5. Ongoing support and follow-up will be provided based on your individual needs.
   
    

Diabetes is a long-term condition that causes blood glucose (sugar) levels to become too high. There are several types of diabetes:

Type 1 Diabetes
Type 1 diabetes is an autoimmune condition in which the body's immune system attacks the insulin-producing cells in the pancreas. It is usually diagnosed in childhood or early adulthood and requires lifelong insulin treatment.

Type 2 Diabetes
Type 2 diabetes is the most common form of diabetes. It occurs when the body does not produce enough insulin or cannot use insulin effectively (known as insulin resistance). It can often be managed through healthy lifestyle changes, medication and, in some cases, insulin.

Gestational Diabetes
Gestational diabetes develops during pregnancy when the body cannot produce enough insulin to meet the increased demands of pregnancy. It usually resolves after childbirth but increases the risk of developing Type 2 diabetes later in life.

Please note: While there are several types of diabetes, this service provides specialist nursing support for adults with Type 2 diabetes only. Type 1 diabetes and gestational diabetes are managed by other specialist services.
