Fothergill Ward (Newham)

When a patient is medically optimised (ready for discharge) from an acute hospital, ward staff will discuss with the patient and their family what help they will need when they leave hospital, either in the short-term or the long-term. There are a number of pathways available, such as going home with a care package or admission to another setting if required.
