Isle of Dogs CMHT
Community mental health teams (CMHTs) are multi-disciplinary teams focusing on the care of people with severe and enduring mental health problems.
The teams provide early assessment, comprehensive programmes of treatment and continuing care for clients.
The objective is to reduce relapse of illness and admission to hospital, and to enable people to remain at home, thus improving their quality of life.
Primary Care health and social care professionals can refer to clients to the community Mental Health Team in their catchment area.
