Tower Hamlets Community Therapy Team

Community Therapy Team (CTT) is one of the community services run by East London NHS Foundation Trust. 
Our team provides specialist Physiotherapy and Occupational Therapy assessments, treatments, and ongoing support – all delivered in your own home environment.

What we do
We work with you to improve your physical health and wellbeing through movement, exercise, and personalised rehabilitation. Our goal is to help you regain or maintain your independence and make daily life easier. Download our patient information leaflet.

Depending on your needs, we can:

  • Support you with safe movement and mobility
  • Help you improve strength, balance, and confidence
  • Share practical strategies to make everyday tasks easier
  • Recommend or provide equipment and aids to support your independence at home

Our team includes:

  • Rehabilitation support staff 

The team members work closely with the Reablement Service, Admission Avoidance and Discharge Service, your GP, the Extended Primary Care Teams and other support services in our community.
 

We expect you or someone important to you to take an active part in your assessment and in identifying what you need to return to independence or to plan your future care.

We also need you to participate in your agreed rehabilitation programme and to be welcoming and respectful to our staff. 

You may be contacted to share your views on the service. Close friends and family who have been involved would also be welcome to share their views. This will help us understand what we are doing well and make improvements to our service.
