Tower Hamlets Community Tracheostomy Service

We work in partnership with all community teams, primary care services and secondary care partners to offer specialist treatment and advise with regard to the altered airway.  The service accepts any patient with an altered airway (tracheostomy or laryngectomy) living within Tower Hamlets.

The team aims to facilitate the discharge process from the acute services, support the discharge once the patient is in the community and then work with all partners involved to ensure effective, safe and appropriate treatment in the community environment.  

This service is not a rapid response service, but as prevention and forward planning is essential in tracheostomy care, strategies and plans are put in place to assist during an emergency situation.  

The service acts as a resource for professionals of all disciplines, patients and carers, to problem solve, advise and support appropriate outcomes for this small, but very specialist patient group.

NB: There is no specific referral form for this service or subsection on the SPA referral form.  Please highlight the tracheostomy need clearly throughout the form and tick District Nursing need.  The referral will be then be triaged to the correct location.
