Tower Hamlets Primary Care Mental Health Service

The Primary Care Mental Health Team are a team of mental health trained nurses working in GP practices across Tower Hamlets.

They support people who have received mental health services but who are well enough to be discharged back to the care of their GP. They also support people who are experiencing mental health issues to support them, in partnership with their GP, to address problems and issues at an early stage.

Your GP can refer you to the team. They will remain your primary health care professional.

After referral to the service, you will be offered an appointment every three months for 12-18 months, until you gain confidence and independence in managing your mental health. You can book an appointment with your GP any time too.

The service can support people with a diagnosis of depression bi-polar disorder, schizophrenia and other mental health conditions.

The team will work with you to identify your goals and work with you to improve your mental health and wellbeing.
