ADHD Self-Assessment for Patients

Have you received a text request from “NHS No Reply”?
ELFT has developed a secure web app tool to collect essential information from people currently on the ADHD waiting list. As part of this process, we will be contacting you via text message.

The text message will come from ‘NHS No Reply’ and will say:

This is the ELFT ADHD service, we are updating our waiting list and need to confirm you still need to be assessed. Please complete this self‑assessment

This message will contain a link which you can view here - https://adhd.elft.nhs.uk/patient, plus your secure personal access code.

Please follow the link provided in the text message and log in using your personal access code.
You will then be asked to enter your date of birth and postcode to verify your identity.
You will have three attempts to enter this information correctly.

If you are unable to access the secure web app after these attempts, please don’t worry - the system will show us that the online form was not completed. In this situation, we will automatically send you a paper (postal) version of the questionnaire.

Once logged in, you will be taken to the secure web app landing page, where you can tell us whether you still need to continue on the waiting list and complete your self-assessment questionnaire.

 

This questionnaire helps us understand your current needs and the impact that potential ADHD symptoms may be having on your daily life.

  • It should take around 15 minutes to complete.
  • You can return to your assessment at any time using the original link in your text message.

Reassurance About the Message
We want to reassure you that:

  • The message is genuine and part of our official ADHD waiting list update process.
  • Completing the questionnaire helps us plan your care appropriately.
  • Some people may not be able to access or complete the questionnaire digitally, and that is completely fine.

     

If you are unable to open, access, or complete the questionnaire:

  • This will not affect your referral.
  • This will not affect your place on the waiting list.
  • You do not need to contact the team.

Our system will automatically show that you have not completed the online form. The team will then contact you, and a postal version of the questionnaire will be sent to you.
