Newham Transitional Practice (NTP) received the ‘Commissioner’s Award – Improving Population Health’ at East London NHS Foundation Trust’s (ELFTs) annual Staff Awards.
NTP is a GP surgery that delivers comprehensive primary healthcare services to residents who have experienced difficulties registering locally with a GP. Colleagues at the practice have consistently worked to reduce health inequalities and provide accessible and equitable care for marginalised and underserved groups – such as those experiencing homelessness and those with substance use disorders.
Awards judges noted that the practice goes ‘above and beyond standard healthcare delivery’ to address the social determinants of health, provide trauma-informed care and build trust.
This year, the Newham Transitional Practice celebrated its 25th anniversary of supporting patients facing barriers to accessing traditional primary care services. The site joined the Trust in 2011, with key achievements including:
• Receiving a ‘Good’ Quality Care Commission (CQC) rating at the site’s most recent inspection.
• Winning the Non-Clinical Team Award from the Royal College of General Practitioners (RCGP) in 2024.
• Recording above-average results in the 2024 National GP Patient Survey, including 97% of patients saying their needs were met during their last appointment (national average – 90%).
Commenting on Newham Transitional Practice’s award win, Louise Wilson, Practice Manager, said: “Improving population health isn’t just about treating illness, it’s about tackling inequality, rebuilding trust, and creating systems that meet people where they are. It’s about listening before we act, and designing care around real lives, not statistics.

Services, teams and individuals who work in east London, Bedfordshire and Luton won awards across a host of categories at the 2025 ELFT Staff Awards.
The evening was funded by not-for-profit community interest company Compass Wellbeing.
The ceremony was held at the popular Art Deco music venue, Troxy, in Whitechapel. The room was packed out with over 500 people who gathered to celebrate the achievements of their colleagues.
The ceremony began with a performance by the Trust’s #ELFTin1Voice choir, comprising of staff, service users and carers.
Lorraine Sunduza OBE, who is the Trust’s Chief Executive, and Paul Binfield, Director of People Participation, compéred the event.
