As part of the service, we are also supporting patients with a diagnosis of Cancer. The Macmillan Care Navigators will support patients who have been newly diagnosed or are already receiving treatment. The Care Navigation team will support the patient along that journey and help to enable effective communication between agencies which form the virtual and informal “team around the patient” including family and informal carers.
Care Navigator (Tower Hamlets Community Health Services)
The Care Navigation service provides support and coordination for patients who have complex health and social care needs. It aims to navigate the patient through the health and social care system and help to enhance the communication with patients, carers and other professionals and services the patient comes in contact with, empowering the patient along the journey.
Referrals are accepted from Statutory and non-statutory services and patients can self-refer.

CHS Care Navigation - our referral criteria:
- Frequent visits to GP and/or request for home visit
- Complex care needs including end of life care
- Patients who self-neglect or at risk of neglect and hard to engage
- Patients who could be taught to self-manage
- Patients with social issues that impact on health
