Who we are

The key function of the Extended Primary Care Service is to treat people in their homes through the establishment of integrated multi-disciplinary teams, working across two localities with strong inter-disciplinary relationships and broader partnership arrangements with other service providers. Intervention and treatment will be pro-active and based on the utilisation of a range of diagnostic tools and referral criteria to ensure the fast track and efficient use of the community-based resources.

The extended primary care team services Tower Hamlets, primarily as two teams North and South encompassing then North East and West Locality, South East and West Locality respectively.

Housebound patients are seen at their homes.

GPs or other healthcare professionals e.g. DN/ Physio, Wards, Social Care, ELFT NHS Partners(TH CCG, Barts Health, GP Care Group)
