Care Navigators work with people with multiple and sometimes complex needs

This service provides comprehensive assessment and care to patients who are housebound and/or at risk of hospital admission, offering supported discharge from hospital and rehabilitation at home and to people at the end of their lives.

The Children’s Physiotherapy Team provide health advice for Children and Young People (CYP) known to our Service, who have difficulties accessing the curriculum due to mobility, functional difficulties and postural needs.

Family Ambassadors are part of the North Central East London CAMHS Provider Collaborative. We work with families and young people living with Autism Spectrum Condition and/or learning disabilities who have been admitted to an inpatient unit.
