The key function of the Referral and Assessment Team (R.A.T.) is to provide a holistic initial assessment of all new episodes of care referred to the Extended Primary Care Teams (EPCT) – Planned Care.
Referrals to CAMHS can be made by yourself, your GP, your social worker or a teacher on your behalf. Young people under 16 may need the consent of an adult they trust. We will speak to you about this when you make the referral.
The Neighbourhood Care Team consists of 12 community nurses who look after patients with nursing needs.
