The City & Hackney Dementia Service (CHDS) is a new model of dementia care which provides a diagnostic service that holds patients from diagnosis to end of life and ensures everyone living with dementia has a timely diagnosis.

Hackney’s Integrated Learning Disability Service (ILDS) is an integrated multi-agency, multi-disciplinary team, providing specialist health and social care support to adults with Learning Disabilities (LD).

The service is designed for City & Hackney residents whose mental health and substance use needs are closely linked and who would benefit from coordinated, specialist support.

The Clozapine clinic is for service users who have been prescribed the drug Clozapine.
